The opportunities for defects are in the product whether you test them or not. Ultimatley, it is the customer who really tests the products. So one might argue that whether a company does internal inspection or not is not relevant. You can still count the opportunities for a defect with a certain product and then see what defects you are getting in the field from the customer. Of course, not using internal inspection may result in more defects being sent to the customer.
The other important point is that the DPMO metric is useful as a comparator. Recently, I heard a six sigma user use the fact the the number of opportunities for defects was large for his product as a justification or excuse for why his defect levels were high. But of course, the customer does not nor should not care about how complicated the product is. That is the supplier's problem.
