Has anybody seen a complaint pyramid - analogous to the safety pyramid?
In the safety pyramid, statistics show that about 15,000 unsafe acts result in about 300 near misses, which result in about 30 first aid incidents, which result in about 1 recordable injury. The theory is if you reduce unsafe acts, you reduce recordable injuries.
I am looking for any type of general statistics that can show the same type of pyramid of values for customer complaints. In other words, for every 1 customer complaint, there is likely XX number of complaints that do not get communicated, XXX number of defects that nobody notices, XXXX number of opportunities, etc...
Has anyone seen anything like this or can point me in the right direction?
