Six Sigma and Organisational Culture

I know this sort of request does not often get a good response, but if you can please spare a little of your time to help a colleague!
I am investigating the effect of implementing Six Sigma on an organisations culture. It is often said that the right sort of culture must exist or be created for Six Sigma to be truly successful. My argument is that if Six Sigma is implemented in it's complete form, i.e. as a Management Philosophy rather than as isolated process impovement teams, then this will go a long way in creating a Quality Culture.
I am looking for organisations that have and have not implemented Six Sigma to help me in completing a questionnaire that assesses the degree of Six Sigma implementation (completed by the Quality Manager or similar), and also measures cultural attributes via perceptions of normal practices (completed by a sample of employees). I will obviously reference any organisations that have helped with my research, and will also be happy to supply a copy of my findings.
