Gage R & Rs/Measurement System Validation

I'd like to hear some opinions on the "necessity" of gage R&Rs in operational and transactional process improvements. When I went through black belt training, our instructors told my class "thou shalt always do a gage R&R" Don't get me wrong, I believe that gage R&Rs are important, you want to be as confident in your data as you can possibly be, but is doing a gage R&R appropriate in every situation? Is it appropriate when looking at defects on completed forms? Is it appropriate when using a digital pressure gage on the shop floor? Are other methods such as gage bias or linearity used in the measure phase to validate measurement systems?
