Leadership Must Play a Critical Role in Define Phase

It is time for organizations to put the D back into leaDership. This means that the D (Define) in DMAIC should be a leadership responsibility, because leadership involvement is key to the success of Lean Six Sigma. The best way to get involvement is to make leadership responsible and accountable for results. Without strong leadership involvement and ownership, it is hard to expect the best results from Black Belts.

Setting Distribution Expectations
I'm getting into a project, and I am not sure of what to expect for the data distribution. We mix chemicals (about 100+), and when done test the pH of the mixture. Refresher: pH is a measure, on a log scale, of the hydrogen ion concentration. The scale runs from 0 to 14 (i.e., is bounded).

"For the actual measurements, should I expect they would be normally distributed, or some other distribution? Bottom line is that the measurements are what they are, but is there any fundamental reason to expect a particular distribution?

Usefulness of G-charts
I was referred to a couple publications from Wheeler and Benneyan talking about g-charts as superior in ultra-low defect rates so that it turns it over as 'units between failures' (as data points increase = better). Of course it needs a whole different UCL and LCL calculation. Does anyone have any real experiences with this stat? Does anyone see this as worthwhile?

Counting Opportunities
We assemble cables and one of our processes has to trim a multicore cable to meet several inspection criteria. The outer sleeve has to be removed a certain distance from the end, the cut should penetrate any of the sleeves of the inner cables, and the outer sleeve has to be removed cleanly without any jagged edge (not cut all the way around).

"My question is: Can I assume for a six core cable there are eight opportunities? Six for cutting any one of the inner core cables, one for the length of the unsleeved part of the cable and one for cutting all the way around?

Calculating Effectiveness
We are working on an MSA attribute study (cross tabulation kappa coefficient method). We are facing a problem in calculating effectiveness of measurement system. Can anyone help us in concluding kappa measurement? If possible please send a complete study as an example.
