My first question would be did you just start discovering that you had batch record errors or did the rate suddenly go up. If it suddenly went up, I would determine what changed that casued it to go up.

I have never seen the case where there was truly only one operator. In some cases, I didn’t find out about the other operators until after I had done the R&R. I then had to go back and redo the study. Now that will really turn off people.

I always have more than one operator. In some cases, I have been one of the operators. In that way, you get data that will tell you if the operator will make a difference.

I’m glad that you are thinking about it. It is very frequently ignored and usually the project suffers. You can however, go to far to the other extreme and get so bogged down in human factors that you never complete the project. You should get input from as many people as you can. To keep from writing a book, I left a lot of things out. The internal and external customers were involved at all stages of the project. There were certain laws, regulations, and agencies were also taken into consideration. There were problems with other departments within the company and they were also dealt with.

I had a Six Sigma project that involved a cleaning system. Samples were taken after they had been cleaned. The samples were cleaned with a very strong solvent and the solvent was captured. The solvent was then evaperated and what was left was weighed. This was the material that hadn’t been removed by the cleaning process.

Six Sigma is supposed to be customer focused. Who is the customer for your department? What are the most important things to that customer? Of those, which is the customer the least pleased with? How can you measure the departments performance to that customer requirement? That’s all there is to it.

Do a search on Google for Six Sigma Report. I did that a few weeks ago and got several thousand hits. Out of the first few hundred, I got several large service type companies with some information on how hey implimented and what results they have reported.
