    Simple… defined as:

    1. Dependent on the person asking the questions….. Which means dependent on the person’s experience or brainstorming.

    3. Highly variability of the output is acceptable…. Which means no MSA on the investigative process would pass

    4. Steers a person to one primary root cause….. no such thing (removing just one…[Read more]

Have you defined how these trained people will be utilized? Are
departments required to complete a project per yellow/white belt
trained? If the answer is no to the first question or yes to the
second question then stop and reevaluate. Projects should be established by company need, not training
metric need. If you decide to train all so…[Read more]

  • Vallee replied to the topic Safety Committe in the forum General 11 years, 10 months ago

John,Did not miss the point, just did not talk about the process yet. So
lets talk about the process metrics that management are driven by:Safety:
    1. Lost Time Incident Rates (a lagging indicator, usually only at the
    corporate level)
    2. Recordable Incident Rates (same as no. 1)Process Metrics:1. Production completion (lagging indicator)
    2.…[Read more]

Carlos,Something appears to be missing here and it looks like you are
have set up up a blanket approach on what to work. You have two
issues as I see it: where the safety and process champions come
    from (by the way, constant turnover has never helped anything);
and what projects you target. Not only is picking the manager with the worse…[Read more]

  • I have seen posts of yours in the past and have been enlightened. It is
not a food chain…. maybe a big dog on the porch world with some
bark and some bite and a little pack mentality. But in the end WE all
learn, revalidate, or sometimes assume just to have to repost the
revision. If you don’t think so… try to find an old post about…[Read more]

  • Read a little closer and you will see that I did not say that you can
not use a process behavior chart (ImR) for arrivals and departures.
What I do say is that you probably need to keep different PBC’s for
distinct departures and arrivals if they are extremely different. In
your example provided it is okay to have such a chart, In Marci’s…[Read more]

  • Wheeler had to make the assumption that “By comparing the
counts directly, we are essentially assuming that the number
of vehicle miles driven in Tennessee in each of these 25 years is
approximately the same.”So he had to ignore that more people continue to move to the
outskirts of towns as cities develop. He had to ignore that the…[Read more]

  • When do measurements of the part really matter? When does
contraction or expansion have the biggest impact:1. Before the weld or after the weld?
    2. What happens if the part heats up or gets cold in the field?
    3. What is the impact on laminates when you heat them after it
cools. Your measurement system should address when the part should…[Read more]

  • Marcy, Within the forest of information, you received a few good trees to
work with so I am going to avoid the math discussion. I want to
get back to the forest. What do you really need to measure and why? You can not take one

  • From the sounds of it and without personally observing your
process, you have a hands on error issue that would be occurring
no matter how you grouped the work. Lets just focus on the three major defects you mentioned below.
First question is whether the defects occur during peak times, on
certain shifts, or it really does not matter…[Read more]

Last response and then it is no use to continue. You have a car I hope. There are different major components on
the car that are joined together. The car is used to get from point
a to b. Sound familiar? Now do you change and inspect all the components at the same
interval.. no. Why ? Because they all have their own OEE and should be…[Read more]

So we have two different machines and we would lower the
effectiveness of one based on the other being a piece of junk? It is
not narrow mindedness, it is understanding the business.Which machine costs the most? Which machine breaks down the
most? Which machine takes more downtime to repair? Which
machine can produce more and is held back…[Read more]

Lim BT,I do not facilitate six sigma projects anymore but spend my days
teaching root cause analysis and facilitating investigations from
