We have used all of the methods you listed.
I suggest, consider using all three methods, if time permits.
Your team will have to think through the objectives, scope, the deliverables and targets for each of the approaches.. For surveys, be sure to use all of the principles of effective survey design and implementation.

It seems, you may need to revisit the scope of your project. To know that something is truly “uncontrollable” is of benefit. Work on the controllable variables and gain as many efficiencies as as you can.

To be successful, Six Sigma has to start from the very top of the corporation.
Your COO needs to attend an Exec. Overview session to learn about six sigma and then think through how he/she wants to deploy six sigma. -

You are correct.
I believe, there is no organization in the world which has become a “six sigma company”. No company has achieved a six sigma level of performance in all of its processes. Six Sigma is a journey.
