you have to first fix your measure & then calculate the DPMO based on the deviation with repect to the measures .
As far as often changing user specs is concern then you have to update each time a measure is changed .

Use run chart for each critical dimensions of different parts. Collect the data in different shifts and using different tools . Calculate X-R charts control limits from collected historical data and again monitor that controls limits with respect to specification limits.
