    @Straydog,

    Thanks for the reply. Is the QFD still an effective tool when developing a product for a single customer? If you have a fuzzy customer requirement, wouldn’t the first step be to reach out the customer to accurately define, i.e. durability = >1 million cycles? IF they come back with a nebulous answer I would then use the QFD for it. Does my thought process make sense?

    Thank you for your response. You are correct, if we were using RTY we could clearly see the seriousness of the issue.

    I have implemented a data collection sheet with the assemblers to record the occurrence. While this is being collected I am running MSAs on the measurement systems used upstream that affect this issue.

    The previous manager of this value stream left 2 years ago. Under his guidance, he would allow the assemblers on the line to “fix” the symptom, which ultimately could have lead to other problems. Since then the new VSM is more focused on the quality of our product vs. meeting his shipment dates. As a result, this “rock” was discovered.
