    Hello M. Rao,
Capturing COPQ in a form is a difficult task because nobody likes to report it. The reported COPQ is generally like the visible part of an iceberg.
I worked at one company where each department head was trained in capturing and reporting COPQ. They then began reporting it to the best of their ability. The result was astounding, and the Director proclaimed that the company could not survive at this rate.
He demanded that the department heads immediately reduce the COPQ by 20%. Some were clever enough to understand that the reported nothing 2 months ago, a good figure last month. What should they report this month? How about 25% less and garner kudos from top management?
Anything that is a problem should be addressed. A majority of COPQ is from chronic problems, that only big changes can cure. The result of a successful COPQ program is increased profits, often for reasons that the management team cannot explain.
