Has anyone done a project on reducing reoccuring delays in discharging patients and thereby reducing length of stay? I’d love to hear some success stories!

I was looking more for delays before the discharge order, i.e; discharge planning failures, procedures and testing back logs, rehabilitation delays…reducing avoidable delays so that a patient can be dsicharged in the expected length of time.
