Can anybody give me some case studies on Application of FMEA ( Failure Mode Effect and Analysis) to desing as well as for process.

component name:wiring
failure mode:broken,frayed,frpm lamp to plug
cause of failure:fatique,heat,crelessness,childbite
effect of failure on system:will not conduct current;may generate heat,blow breakers;or cause shock.
correction of problem:Use of wire suitable for long life in exterme environment anticipated.
comments:Dangerous;warning on instructions

Failure Effect (how the above will affect us) paying premiums in error; paying claims in error; suppliers giving us false error reports; query results would be in error; former employee receives matching 401K contribution and should not; too much pension service credit.
Potential Causes: each of the failure effects have several causes…I’ll focus this example on just one of them…paying premiums in error. Managers not properly trained in how to fill out a termination form; data entry operator enters incomplete/incorrect data into the system; HR did not complete the action/reason codes correctly; our termination form does not accommodate multiple transactions; “special” severence benefits promised and should not have been; incorrect corrections to job record
So you begin to see as you go right on the FMEA form the form expands as you add more and more issues.
Existing Controls: what is in place now to “catch” these potential causes? Again on the previous causes we could answer variable and inadequate, manual, none
And then the rest of the columns like recommended actions and responsibility is up for your team to decide for each point. If you would like to post your e-mail address, I would be happy to e-mail you an example of a completed FMEA for one of my current projects.

sridhar0December 19, 2001 at 1:01 pm #70682Don’t waste your valuable time conducting a FMEA. A good brainstorming session with the project team, followed by nominal group technique, then Cause and Effect Matrix will do the same job in a fraction of the time. Controversial, but true.

I would appreciate a copy of one of your FMEA case studies. We had a similar situation where our company failed to identify “old terminated” employees and therefore continued to pay insurance prermiums, etc…

Other FMEA applications include machinary FMEA, System FMEa . Machinary FMEA is a very importnat part of TPM imnplementation also.

Following is an example of a simplified FMEA for a seat belt installation process at an automobile assembly plant:
S. PoO PoD RPN
Wrong color S.Belt 5 4 3 60
S.B.Bolt Not Tighten. 9 2 8 144
Trim Cover Clip Misali. 2 3 4 24
As you can see,three potential failure modes have been identified.Failure mode number two has an RPN of 144,and there is therefore the highest priority for process improvement.
