    Dectectability needs to be broken down into 2 types.  The 1st type is the chance of detectability of the flaw before the customer sees the product and the 2nd is the chance of the customer detecting the flaw after they have the product.  To measure these, you are going to have to know your product and you are going to have to know your customers.  A simple statistical analysis of any QA inspections you do on producing your product will account for detecting flaws before the customer sees them, and a statistical analysis on cutomer complaint information should tell you the types of things the customer is seeing on their end.  You may not be able to see flawed tubing, for example, by a visual inspection, but when a fluid under pressure within the tubing is applied, and the tube bursts, the customer will detect and if serious enough, will contact the manufacturer.  Since you are in the healthcare business, errors in equipment can have expensive consequences, so you will need to be careful.

    I think you should refer the “Failure Mode Effects Analysis (FMEA) ” Manual ,Which is a one of the seven pack manuals of QS9000. It contains certain guidelines using which you can actually quantify the detectability of your quality system against any process failure . Detectability is not same for all the process failures.
