    Hi,

    Can anybody share filled in FMEA sample for software support & maintenance project to identify risks in the process (for eg: Incident management process).

    Lakshmipathy.B I understood the request. I don’t understand why you think you can roll into the DF and expect people to run to your request as if we owed you an answer. Google it. Open up Excel and make your own spreadsheet. If you want me to do that for you send me a PO and a cheque.

    @pravin25 even if someone provides a filled-in FMEA for a similar process it won’t help much. Discovery of what’s actually happening, or could happen, in your particular process is a prerequisite for FMEA. Even if you have a defined step-by-step process that may not be what actually happens. Start by putting together a team of people who actually do the work to discover what they actually do. You might find that you have a poorly defined process. You’ll need to fix that before you can do FMEA. After that, your process is YOUR process. Risks that others have identified in similar processes and how they’ve evaluated them will be more likely misleading than helpful. You need to do the FMEA team exercise yourself.
