    Does anyone have a root cause analysis procedure for their company that they would not mind sharing?  I have to create a root cause OP.  There is so much information out there.  I am in the medical device human biologics field.  I do not want it too detailed because then it would not be followed.  And I dont want to start from scratch.  I bought the one from sixsigma forum but I am looking for something a bit better.
    My other question is where you document the root cause for a non conformance?.  For example if someone does a 5 Why.  How does one document it?  Any help would be appreciated.

    Write a guidance document, not a procedure.  Every problem is different and most people responsible for solving problems won’t read the damn thing anyway and you will end up with gaps between your procedure requirement and your actual investigations.
    The best way to ensure you get the documentation you want is to give them well designed and easy to use forms to fill out.  Make all of these forms optional attachements to your normal CAPA forms.  If you do a good job, people will use them.  If you don’t…
    The best 5 Why’s form I ever saw was pretty simple.  It had the problem statement in a box a the top, a box for Why 1 below that, a box for Why 2 below that, etc. etc.  It had a second page where you could put additional Whys.  The bottom of the first page had “tests” (yes/no questions) that personnel could use to determine if they had hit the root cause:  Example –  Can you turn the problem on and off by turning the root cause on and off?  It was developed by Pratt & Whitney.
    Again, keep it simple and keep it flexible.  The best way to improve the quality of RC analysis you receive is to ask your respondants to “prove it.”  Don’t close your issues out until they do.

    So far the defect rate stands as follows:
    9 requests for the free stuff offered by Vidya. 5 requests included an email address, 4 requests included no email address. This is a 44.4% defect rate or a sigma level of 1.64.
    Any bets on whether that rate gets better/worse or stays the same?

    This is a short term rate.  Wait a few years and I bet the defect increases.
    Now, if you are able to determine the distribution of where in the world these requests are comming from, you could possibly apply HBGB’s Continental Shift Adjustment Algorithm in a weighted manner to get an accurate estimate.
    Can you send me your minitab worksheet that details the analysis?
