    Has anyone done a project on Sitter utilization/reduction?

    Brit,
    We haven’t done any project in particular but we sure did re-write our sitter policy to minimize the expense that occurs due to sitters. Mostly its the staff decision, if they see high-risk/fall patient they request the attending for a sitter order and we changed the sitter policy accordingly but were not able to control any siter hours.
    Alternatives considered in order to control the costs included conidering volunteers, family members etc to be sitter, but I dont know that changed anything in the number of hours. Besides that we added some more duties to sitters besides just sitter function.
    If you have any other thoughts on trying something new please do share it, we are also facing the problem with the number of hours we add towards sitters.

    We are doing the same in terms of adding duties to the sitter function. We are also removing non-sitter staff from the sitter responsibility (or looing at paying them at the same rate as a sitter).  This should show a cost reduction of about $78,000/yr.  We are insituting a rigorous training and orientation for the sitters – but that won’t help us reduce the usage of them – might actually increase it.  Heard some people were looking at virtual monitoring, but I am a bit skeptical.
    Hopefully others will have seen some innovative ideas out there.

    I am in the control phase of a sitter project where we reduced sitter use by 30% on our pilot unit.  We developed criteria for a sitter and a decision tree that the nurse must address each shift when requesting a sitter.  Please contact me if you would like further information.

    Debra,
    Where are you located?  Size hospital? Would you be willing to share your results?
    I am a fairly new black belt at SJH in central WI and will be mentoring a green belt that has the project of decreasing sitter/observer utilization.  One of our thoughts is to create an algorithim along with criteria medical staff can utilize for USING a sitter.  Would like to keep in touch if your project was similar.     The fall reduction committee at my institution is looking to develop a decision tree to accurately determine the need for a patient sitter.  Would you be willing to forward the information that you have for us to review?  It would be appreciated very much.

    Virtual monitoring is great to reduce critical care costs (e-icu).  However that is another topic. 
    We are using non-sitter staff instead of agency.  RN as a sitter is still cheaper than agency some times.

    Ram,
        Thanks for sharing your thoughts and ideas with this group. Judging by the overwhelming amount of requests for help and additional information that you’ve received, I think it is safe to say that most of us are in the same boat. I work in an acute care hospital setting and am experiencing the same dilemma. I am intrigued by your action plan. I would appreciate any assistance you can give me. I would like to learn more about your decision tree and what criteria is set for this group in order to justify the sitter use. It would be interesting to see the algorythm you developed for this initiative. Please share!
