Urgent: Please help : Deployment flow-chart

Hello,
Very interested in learning more about the deployment flow-chart (By listing the activities on the vertical axis and the groups on the horizontal axis at the top, and then showing the flow of activities using the rectangles is a great way to illustrate the hand-offs in the process).
If someone has a template and can share, that would be great!!!
