Nelson, Good morning!
Regarding your request about Kano analysis — I’m not quite sure what you need regarding a “template” … could you elaborate?  The Kano model is a “thinking” tool rather than an strict numeric analytical tool.  I don’t think I have ever seen anyone use a template when applying the Kano model.
 
In the meanwhile … FYI, there is a good tutorial about the Kano Model (with animation and voice-overs) that can be launched at this link:http://www.c2c-solutions.com/kano_tutorial.htmIt includes good demonstration of different types of customer needs, including:- Basic Needs; – Performance Needs; and- “Excitors”The tutorial is a little “commercial” at the end, but otherwise is quite well done…. hope this is helpful to you ….
